Full Time
Monday - Friday 8-5PM typical, with some weekend and evening hours as needed.
Job Duties:

1. Oversees and coordinates aspects of the RH supportive housing program.
2. Responds to referrals of new clients in timely manner, including completing the intake form and assessment of the client’s needs.
3. Keeps client case files up to date and organized.
4. Maintains current/accurate client data and keeps detailed case notes in Foothold database.
5. Communicates regularly and effectively with the client, service providers, and support personnel to ensure that clients meet housing application requirements and maintain housing when secured. Provides information and assistance to clients after placement to help maintain housing.
6. Assists in identifying adequate housing options for clients that fulfill their specific location, size, and affordability requirements.
7. Conducts assessment of identified housing to ensure compliance with HUD requirements.
8. Assists clients in understanding and signing lease agreements, as well as, providing support in completing applications and providing necessary documents for applicable housing vouchers/assistance.
9. Submits rent payment based on pre-determined assistance.
10. Be available to respond to questions from clients and authorized service providers/family members so stable housing can be maintained.
11. Maintains professionalism in representing Recovery House/CCS to landlords/property managers with the intention of expanding the network of properties which house people with disabilities.
12. Identifies opportunities for housing advocacy and collaborates with Housing Authority and Director of A&D Services in pursuing them.

Documentation and Quality Improvement:

13. Maintains case record documentation that is complete and in compliance with HUD requirements.
14. Maintains appropriate documentation/receipts for expenditures.
15. Case management and recordkeeping includes, but is not limited to: housing intake; service planning; service delivery including advocating for clients with property owners/managers as necessary and providing resource information; and case closure.

Interpersonal and Leadership Skills:

16. Responds to clients needs in an appropriate and timely manner.
17. Demonstrates appropriate interactions with coworkers that promote a pleasant work environment.
18. Maintains client confidentiality at all times.

Support Duties:

19. Transports clients as assigned in a safe and timely manner. If using a CCS vehicle, signs vehicle in and out per agency policy. Completes inspection and documentation prior to driving. Ensures that vehicles remains clean and in good repair.
20. Identifies and reports to the Director of A&D Services or his/her designee in a timely manner any safety concerns.
21. Completes additional job duties assigned by supervisor.
Requires excellent organizational and interpersonal skills.
Requires satisfactory completion of CPR prior to reporting to the work site.
Requires satisfactory completion of CPI within the first 30 days of employment.
Requires ability to be covered under company insurance to drive a company vehicle for client transportation.
Requires valid driver’s license and current auto liability insurance.
Requires ability to transport clients in a private vehicle when necessary.
Requires completion of annual training in compliance with DMH standards.
Requires thorough knowledge of the 12-Step Model of Recovery.
Previous experience in an A & D treatment setting strongly preferred.
Associates Degree
Associates degree required; Bachelor’s degree preferred.
